Supported Living Services (Adult Foster Care)

Community Involvement Programs provides Supported Living Services for adults with disabilities in adult foster care homes. These small group residences are home to four people; each person has their own bedroom. Homes in Hennepin County are located in Richfield, Golden Valley, Hopkins, and New Hope. In addition, we operate six homes in Pine County. Each home is licensed as adult foster care, and services are funded by the Medicaid wavier. Staff at each residence provide supervision and training based on each person’s individual needs and plan.
