Support Planning
Most consumer-directed options require that the person or his or her representative develop a community support plan. This plan must be approved before services can begin. The best way to develop a plan is by using a person-centered process. During person-centered planning, the team would consider areas of your life that you would like to change for the better, what kind of help you might need to accomplish this, what other needs do you have now, and what would be the best way to meet those needs. Based on the answers to these and other questions, the community support plan is developed.
Plans need to include details, such as:
- Who will provide the services;
- Qualifications and training requirements for support workers;
- How much support workers will be paid;
- What goods are going to be purchased; and
- An emergency back-up plan.
Support planners are individuals who have experience in person-centered planning approaches. A support planner can help you develop the plan, complete the required paperwork, develop the detailed budget and submit your plan for approval.
Community Involvement Programs cannot provide both FSE services and support planning services to the same individual.
