Service Coordination: CADI Waiver (Hennepin County)

Community Involvment Programs provides Waiver Service Coordination to individuals in Hennepin County who are eligible for the CADI Waiver and have a diagnosis of serious and persistent mental illness. The goal of waiver service coordination is to help people access needed services and supports. Case managers can also help people locate and access health care providers, educational resources, or other social services. Our case managers work with people to develop and implement a Community Support Plan. They also complete assessments, arrange for services, monitor services and consult and advocate on behalf of the individual.
