Case Management: DD Waiver (Hennepin County)

Community Involvement Programs provides case management services in Hennepin County to individuals eligible for the Developmental Disabilities (DD) waiver. Case managers assist individuals to identify and access needed waiver services or other services provided under the state Medicaid plan. Individuals can also receive assistance finding medical, educational, or social resources. Case managers work with the person to develop the Individual Service Plan, solve problems, monitor service implementation, and act as an advocate for the individual.
