Care Coordination: Seniors (Medica)

Community Involvement Programs provides Care Coordination for individuals who are Medica members and elderly. Case managers help with coordination of health care services and can assist in accessing waiver and other state Medicaid services. Services that are provided include the development of the Care Plan and arrangement of needed services. The case manager can help with problem-solving, assessment, and advocating on behalf of the individual.
