Case Management

In Minnesota, most services for individuals with intellectual disabilities and mental illness are authorized by local counties based on an approved Individual Service Plan or Community Support Plan. In recent years, some counties have contracted with community-based providers for case management services. Community Involvement Programs currently provides the following services:

• Service Coordination: CADI Waiver (Hennepin County)
• Case Management: DD Waiver (Hennepin County)
• Care Coordination: Seniors (Medica)
